WACOSS is deeply committed to keeping our people, stakeholders and communities as safe as possible, and we recognise that COVID-19 vaccinations are an essential public health measure to prevent, control and abate the serious public health risk presented by COVID-19. As such, the WACOSS COVID-19 Policy adheres to the requirements of the Western Australian Government’s Care Services order.
Effective 10 December 2021, access restrictions will apply at WACOSS to align with the Chief Health Officer’s Community Care Services Worker (Restriction on Access) Directions (Directions).
Therefore, WACOSS will be requiring anyone accessing our room hire facilities, who is meeting in person at WACOSS to be:
We ask that you please confirm the vaccination status of your attendees by completing this declaration template and emailing it to [email protected] prior to meeting (a copy of your vaccination certificate is not required). The email declaration will be confidentially stored by WACOSS.
Please note, if we do not receive the completed vaccination declaration prior to your scheduled meeting date, you will not be able to access the room hire facilities.
